Our Adult Social Care service is seeking a Team Manager to join the Intermediate Care Southwark (ICS) team. If you are a passionate leader with a commitment to enhancing recovery and making a meaningful impact in the community, this is your opportunity to shine!
About the Service:
Intermediate Care Southwark (ICS) is at the forefront of delivering integrated, multidisciplinary health and social care within the community. Our aim is to reduce unnecessary hospital admissions, expedite early discharge, and minimise the need for long-term care by providing high-quality services directly in people’s homes.
We are passionate about empowering individuals to regain their independence and adopt a strength-based approach, building on existing skills and resources to support recovery. Our service also plays a key role in the Community Urgent Response function, providing vital crisis intervention and proactive community support to help individuals avoid hospital stays and transition home safely from Emergency Departments.
About the Team:
Our brilliant team is made up of dedicated Social Workers, Assistant Practitioners, Occupational Therapists, Physiotherapists, Nurses, Pharmacists and Rehabilitation Support staff.
About the Role:
This role offers a fantastic opportunity to lead and inspire a multidisciplinary team committed to excellence in care. Reporting directly to the Service Manager, you will be an integral part of the ICS management team, where your leadership and expertise will ensure that social work and occupational therapy practices are delivered to the highest standards.
You will play a key role in implementing a strength-based approach across the service, optimising resources, and driving quality in person centred care. Your contributions will directly impact the well-being of those we serve, making a real difference in the community.
About You:
We’re looking for a highly experienced social worker who brings a deep understanding of intermediate care and reablement. The successful candidate will:
- Be a natural leader with a proven track record in multidisciplinary leadership and operational management.
- Have passion for delivering top tier social work practice.
- Showcase strategic vision in guiding our team to achieve the best outcomes for our service users.
