Help Change Lives & Empower Vulnerable Adults Towards Independent Living

Southwark Council is seeking a compassionate and dedicated Supported Pathways Officer to support vulnerable adults in our managed hostel service. In this rewarding role, you'll work directly with residents, providing key support, risk assessments, and resettlement assistance, helping them gain the skills and confidence to live independently.
What You’ll Be Doing
✔ Key working with a caseload of residents, providing person-centred support to help them maintain their accommodation and move towards independent living.
✔ Developing personalised support plans, identifying goals, and linking residents to services that improve their physical and mental well-being.
✔ Conducting risk assessments, safeguarding checks, and ensuring residents understand health and safety requirements.
✔ Working closely with health, social care, and criminal justice services, advocating for residents to get the support they need.
✔ Providing hands-on practical support, from ensuring living spaces are habitable to accompanying residents to key appointments (e.g., DWP, health services, housing viewings).
What We’re Looking For
We’re looking for someone who is:
🏠 Experienced in supporting homeless individuals with complex needs in a housing, social care, or support setting.
🛠 Knowledgeable about trauma-informed care, the Care Act, and Housing legislation related to single homeless adults.
💬 A strong communicator and advocate, able to support and motivate residents while working collaboratively with multiple agencies.
