About the Role

Our Transfer of Care Team is seeking a dedicated Weekend Social Worker to support our efforts from Friday to Tuesday, covering King's College and St. Thomas' Hospitals. You will play a crucial role in facilitating safe and timely discharges, ensuring adult service users receive the necessary support to transition smoothly from hospital to home or other care settings.

Key Responsibilities:

- Comprehensive Assessments: Conduct Care Act Assessments to determine the needs of service users and develop appropriate care plans.

- Capacity Evaluations: Perform Mental Capacity Assessments to ensure decisions made are in the best interest of service users who may lack capacity.

- Collaborative Care Planning: Work closely with multi-disciplinary teams, including healthcare professionals, to create and implement holistic care plans.

- Safeguarding: Handle safeguarding enquiries and referrals, ensuring the protection of vulnerable adults.

- Best Interest Decisions: Regularly complete Best Interest Meetings and decisions for service users with cognitive impairments.

- Crisis Management: Respond effectively to urgent situations and make key decisions to support the immediate needs of service users.

 

About You

We are looking for a resilient, confident, and enthusiastic social worker who upholds the highest professional standards. The ideal candidate will have:

- Qualifications: A degree (or equivalent) in social work, completion of your ASYE year, and registration with Social Work England.

- Decision-Making Skills: The ability to make sound decisions quickly, often in high-pressure situations.

-Communication: Excellent verbal and written communication skills to interact effectively with service users, families, carers, and multi-disciplinary teams.

- Report Writing: Strong skills in writing detailed, accurate reports and care plans.

- Safeguarding Knowledge: A thorough understanding of safeguarding processes and legislation.

- Empathy and Compassion: A genuine desire to improve the lives of vulnerable adults, demonstrating empathy and compassion in all interactions.

- This post requires a satisfactory Disclosure and Barring Service (DBS) check at the Enhanced level.

- Enhancement pay is provided for weekend and bank holiday cover (Time and a Half!)
