The role

Southwark Transfer of Care (Hospital Discharge) Team South are seeking a Weekend Social Worker covering Friday to Tuesday for our busy team covering Kings College and St Thomas' Hospitals. The team provides support for adult service users requiring assessment and provision of services to support safe and timely discharges from hospital.

Our social workers work closely with the hospitals multi-disciplinary teams to undertake a range of activities to facilitate discharges within the Discharge to Assess framework, including assessing for and arranging nursing and residential placements and generic packages of care. 

The team works with a high number of service users with cognitive impairments, therefore social workers regularly complete Mental Capacity Assessments and Best Interest Meetings / decisions, the team also receive Safeguarding referrals from the hospitals and carryout safeguarding enquiries as required.

About you:

You will be a qualified social worker with good knowledge, and preferably experience, of working in a Hospital Social Care setting, including undertaking person-centred assessments of adults and their families and carers. You will be based in the Transfer of Care (Hospital Discharge) Team, this means you’ll be fully up to speed with relevant legislation and the principles of self-directed support.

Hours of work: 36 hours (Friday – Tuesday).

Enhanced pay for Weekend Social Work role.

Applicants must hold a recognised Social Worker qualification and be registered with Social Work England.

Please note a satisfactory Disclosure and Barring Service (DBS) check is required at enhanced level.
