The Role

We are looking for a qualified social worker with current Social Work England registration to join the START Homeless Outreach Service (Southwark), providing support to adults with mental health needs.

The role will focus on complex case management, working with adults with mental ill health to meet their adult social care needs. To undertake person-centered social care assessments/reviews, direct work with clients to achieve social care goals, safeguarding investigations, and multi-agency working.

The post holder will use assertive outreach and longitudinal relational care approaches in a multi-disciplinary team situated in the South London and Maudsley NHS Trust. They will also work closely with ththird-sectorolleagues, running clinics at homeless day centers and attending Street Population meetings.

This role is part of a multi-disciplinary team located at South London and Maudsley NHS Trust with supervisory support from the team and the adult social care mental health department.

About You

- You will be a qualified social worker with Social Work England Registration with knowledge and experience of working with adults with mental health needs.

- You will benefit from regular supervision, reflective practice, relevant training, and learning opportunities.

Additional Information

- This is a permanent full-time position

- An enhanced DBS check is required

- Standard 36hours, Monday to Friday
