The Role
Southwark Social Care looking for confident and experienced social workers to join our All Age Disability Service (working with children and young people aged 0-25) and also our Learning Disability Service (working with adults aged 25+).
These services provide a comprehensive and integrated social care pathway for children, young people and adults with learning disabilities and other disabilities form childhood into adolescent and on into adulthood, through a seamless care pathway from Children's legislation to Adult legislation and care and support.
As a social worker you will be responsible for providing comprehensive and effective integrated social work service to respond to the needs of children, young people and/or adults with learning disabilities in accordance with Children's and/or Adult legislation. This will include undertaking statutory responsibilities including visits, reviews, reporting to court, and complex safeguarding.
You will be expected to be able to undertake strength based social care assessments, determining the needs of an individual and producing outcome focused support plans. Ensuring focused interventions are delivered to safeguard and empower children, young people and adults with learning disabilities.
An ability to establish rapport and build and maintain a professional, respectful and honest relationship with the service user and their family/carer is key to this role and in developing trust, assisting in safeguarding and promoting positive and timely outcomes for families.
