The role

An exciting opportunity has arisen for experienced, creative and flexible professionals to join the Southwark Mental Health Service Reablement and Care and Support Teams. The Mental Health Service encompass highly skilled multi-disciplinary teams consisting of Social Workers and Assistant Practitioners and Occupational therapists. We are looking for experienced and qualified social workers and/or occupational therapists to join us.

We are based in a friendly office in Camberwell Road and work across the diverse and developing borough of Southwark; from leafy East Dulwich to the urban hustle and bustle of London Bridge.

As part of the Reablement Team you would be focusing on short term interventions with adults with mental health issues, and aiming to prevent or reduce long term need via use of targeted, goal focused pieces of work.

Meanwhile, in the Care and Support Team you would be focusing on offering longer term support to adults aged 18-65 with a primary mental health diagnosis, who are living in the community and are in receipt of a package of care.

Our Occupational Therapy assessments are guided by the Care Act and the Model of Human Occupation, and focus on identifying person-centred goals. We support people to maximise their independence by developing their skills, motivation and confidence through interventions including rehabilitation and adaption. Our assessment and intervention help share the care provision for the individual, ensuring it maximises their independence and meets their support needs.

As an Occupational Therapist you will hold a caseload of approximately 12-15 clients at one time and your work will be predominantly OT focused; undertaking OT assessments and interventions. However, there will be opportunities for you to develop your generic skills through undertaking duty assessments and reviews. As part of the Care and Support Team there was also be an expectation for you to undertake work with complex cases, utilising all the resources of the multi-agency network and the community.

As a Social Worker you offer person centred social care assessments and support through the framework of the Care Act 2014; providing short term interventions and goal focused work, including coaching and support adults with mental illness to develop skills and confidence as part of the Reablement Team, or offer longer term support, holding complex cases and completing care act reviews as part of the Care and Support Team.

We have a robust professionals and management supervision structure and there will be opportunities to develop your skills as a supervisor and a placement educator. We also offer a strong CPD  programme and ensure for Occupational Therapists, they have access to regular meetings and forums locally and with our colleagues in the Southwark Physical Disabilities Occupational therapy services.
