The Role:

 

The Mental Health Reablement Team works with adults with a primary need of mental health for up to 10 weeks to help prevent, reduce and delay needs under the Care Act 2014. The MH Reablement Team are looking for an experienced social worker to work in a strengths-based, rehabilitative approach in order to support adults to become as independent as possible.

As part of the MH Reablement Team the successful candidate will meet regularly with service users at home, in our office and in the community, and will support them to achieve their identified goals. This might include accessing community resources, collaborating with a wider network of professionals, or liaising with family and wider social networks to ensure the service user has the appropriate level of support.

Key duties will include assessment and provision of support under the Care Act 2014, including adult safeguarding; arranging and commissioning packages of care; assessing mental capacity where relevant; and triaging referrals as part of a duty rota. There is also the opportunity to develop skills in specialist areas, and to train as an Approved Mental Health Professional and engage as part of the AMHP rota once suitable experience has been attained. 

About You:

You will have a sound understanding of reablement and rehabilitative approaches and have post-qualification experience of working with adults under the Care Act 2014, ideally within a mental health setting. You will be a strengths-based, anti-oppressive practitioner with excellent knowledge of social work models and theories that you will be able to apply to your work. You will have strong administrative and organizational skills and be able to work alongside a wide variety of professionals.

Relevant social work qualification and up to date registration with SWE required.

Working hours 36 hours per week, 9am-5pm Monday to Friday
