ICS SW recruitment:

We are recruiting to Social Work positions within Intermediate Care Southwark. Intermediate Care Southwark (ICS) is an integrated health and social care team including physiotherapists, social workers, occupational therapists, nurses, pharmacists and rehabilitation support workers who provide short term, targeted support to avoid hospital admission and support discharges by working together to maximise a person's independence.

The ICS Reablement focus includes optimising skills to support recovery. We work from a strength-based position to build on existing skills and resources. The service works alongside specialist care provision. The need for care is regularly reviewed within the period of Reablement. If, after the teams input, the person has ongoing care and support needs, an assessment is completed to determine eligibility for services.

ICS also supports the Community Urgent Response function, entailing crisis intervention and proactive engagement in the community to avoid hospital admission and support people home from Emergency Departments.

The role:

The role provides an exciting opportunity to work in a multi-disciplinary setting, with a focus on strengths based practice and promoting independence and recovery.

The main duties of the role include goal setting, completing Care Act assessments and devising care and support plans, completing reviews, completing Mental Capacity Assessments, chairing Best Interests meetings, crisis intervention, signposting, providing advice and information and working with people with complex social needs.

There is also a Safeguarding function within the service, which includes completing initial fact finding and assessing and minimising risk.

About you:

  • You are enthusiastic and passionate about meeting the needs of the local community and are committed to working closely with those with care and support needs, their families and carers, alongside meeting the aims of the service.
  • You are a resilient social worker, who is a confident decision maker and good communicator.
  • You can prioritise work effectively, manage competing demands and stay calm under pressure.
  • You have experience of working collaboratively in multi-disciplinary settings and are able to develop effective working relationships both inside and outside of your own profession and the service.
  • You have the ability to problem solve, identify and manage risk and have an extensive knowledge, understanding and application of the legislation that relates to adults and older people including Care Act, Mental Capacity Act and Adults Safeguarding.
  • You hold a degree in social work and have completed the ASYE programme, are registered with Social Work England, with evidence of continuing professional development.

 

Additional details

The basic working hours for this role is 36 per week, Monday to Friday.
