The Older Peoples and Physical Disabilities Hospital Discharge Teams are vital in enabling timely and safe transfers from hospital for Southwark residents. With a large emphasis on the ‘home first’ model, the team work collaboratively with colleagues across the acute and community health and social care system to ensure a safe transfer of care. The team are involved in both health and social care discharge to assess processes; they have a wide range of experience, skills and knowledge. The team work closely with colleagues in the acute trusts and community to. We work collaboratively we have successfully transferred as many people home as possible.
The role:
Southwark Hospital Discharge Team South are seeking newly qualified social workers for our busy team based at Kings College Hospital. The team provides support for adult service users requiring assessment and provision of services to support safe and timely discharges from hospital.
Our social workers work closely with the hospitals multi-disciplinary teams to undertake a range of activities to facilitate discharges within the Discharge to Assess framework, including assessing for and arranging nursing and residential placements and generic packages of care.
The team works with a high number of service users with cognitive impairments, therefore social workers regularly complete Mental Capacity Assessments and Best Interest Meetings / decisions, the team also receive Safeguarding referrals from the hospitals and carryout safeguarding enquiries as required.
About you:
You will be a newly qualified social worker seeking to complete your ASYE (Assessed and Supported Year in Employment). You will have good knowledge, and possibly experience, of working in a Hospital Social Care setting, including undertaking person-centered assessments of adults and their families and carers. You will be based in the Hospital Team, this means you’ll be fully up to speed with relevant legislation and the principles of self-directed support within this environment.
