About the Team:
The Assessment & Intake Team is a vibrant and fast-paced team based in the community. We provide an initial assessment function for the Older Persons and Physical Disability Service, carrying out short-term pieces of work (up to 6 weeks). Our team completes Care Act Assessments, puts in place long-term packages of care, undertakes carers assessments, urgent assessments for care and respite, assesses for placement in residential and nursing home settings, and more. We work closely with partner agencies such as the NHS, ICB, housing, and community voluntary agencies, as well as the Occupational Therapy staff within the team.
About the Role:
As a member of our team, you will be responsible for holding a caseload, completing Care Act Assessments, undertaking unscheduled reviews of care, completing carers assessments, presenting packages of care to the funding panel for approval, and using community resources to support service users and their families to maximize their independence. You will also complete Mental Capacity Assessments and Best Interest Decision making, arrange care home placements from the community, stabilize complex case issues for transfer to the longer-term team, and undertake the initial concern stage of Adults Safeguarding.
To be considered for interview, your CV and supporting statement will clearly evidence:
- Proven experience in a similar role.
- Strong communication and relationship-building skills.
- Ability to think creatively in meeting the needs of service users and informal carers.
- Experience in completing Care Act Assessments and Mental Capacity Assessments.
- Arranging care home placements from the community and stabilising complex case issues for transfer to the longer-term team.
- Undertaking the initial concern stage of Adults Safeguarding
- Knowledge of community resources and ability to use them effectively.
