We are looking for someone with experience in healthcare, social work or education to come and join the team, leading on the personalised commissioning arrangements for people with Learning Disabilities, Autism, and Mental Health needs.

In the context of Learning Disabilities and Autism, this will include the SEL Learning Disabilities and Autism Programme. The post-holder is responsible for overseeing the processes around Care (Education) and Treatment Reviews (C(E)TRs) and the Dynamic Support Registers (DSR) for children and adults, improving the services available for people with Learning Disabilities and Autism in Southwark, those with mental health needs, and minimising the use of mental health hospitals for this group.

In a mental health context, this means supporting a select group of residents, with specific mental health needs to receive co-ordinated, personalised and holistic support to effectively meet their needs. This may include co-ordinating aspects of care & support with local and out of area providers. This role provides leadership and outcomes-focused management for commissioning for health and care services by building working relationships and collaboration across the council, the wider health system and through Partnership Southwark.

We are looking for someone who is able to operate within a complex system, who can build excellent partnerships with stakeholders across health, education and social care, and has an excellent understanding of learning disabilities, autism and mental health

Main duties of the job

 

1. To be responsible for the delivery of the Learning Disability and Autism commissioning functions, including the delivery and oversight of Care (Education) and Treatment Reviews (C(E)TRs) and the Dynamic Support Register (DSR).

2. To support the delivery of projects related to the SEL ICB/Southwark LDA Programme, monitoring and managing progress and taking responsibility for problem solving; resolving issues to ensure individual projects and the wider commissioning programme remain on track to deliver on time and in budget.

3. To manage a small caseload of residents with complex physical and/or mental health needs and support their personalised commissioning arrangements. This may include financial management (e.g. co-ordinating any associated purchase order creation, invoice processing, internal reporting) and multi-disciplinary stakeholder communication.

4. To support specific commissioning projects that help shape and influence the provider market with regards learning disabilities, autism and mental health, including links to employment and housing.

5. To develop and maintain expert knowledge on relevant legislation, guidelines and government policy in relation to learning disabilities, autism and mental health across children, adults and families in order to support and develop effective collaboration with service managers from across the department.

6. To work closely with strategy, performance and public health leads to contribute to the planning for the Joint Strategic Needs Assessment and delivery of the Health and Wellbeing Strategy

7. To facilitate stakeholder involvement in service re-design and development in relation to the LDA Programme and mental health services.

8. Where required, to represent departmental commissioning at cross agency strategy, planning and development meetings, and contribute to the development of new inter agency initiatives across the children and adult’s system in Southwark.

9. To chair meetings, including inter-agency, multi-disciplinary meetings as required and ensure that departmental and service user interests are properly reflected.

10. To carry out the duties and responsibilities of the job in accordance with the Council’s and ICB’s Health and Safety Policy and relevant Health and Safety legislation and to actively promote equalities through the application of the Council’s employment practices and procedures in accordance with relevant legislation and best practice guidance.

11. To undertake other duties appropriate to the post that may reasonably be required from time to time

This is an exciting opportunity for a Personalised Commissioning Manager to work in the commissioning team in Southwark. The Integrated Commissioning Team sits across South East London (SEL) ICB and LB Southwark. The post will sit in the Live Well Team, working across Learning Disability and Autism and Mental Health.

The ICB and the Council are part of the SEL Integrated Care System, which has 4 core purposes:

• Improve outcomes in South East London population health and health and care services
• Tackle inequalities in outcomes, experience and access suffered by the residents of South East London
• Enhance productivity and value for money in the in the use of health and care resources in South East London
• Help the NHS support broader social and economic development in South East London.
