The role:
The Care and Support Team is a specialist Mental Health Social Care Team. We provide Social Care support to community dwelling adults with eligible Care and Support needs arising predominantly due to their mental health difficulties.
We work closely alongside South London and Maudsley Mental Health NHS Trust and NHS Primary care services who hold responsibility for the client’s health care needs arising from their mental health difficulties.
As the one of two Occupational Therapist’s within the team, you will work closely with the Social workers and Assistant practitioners to support clients in their ongoing recovery journey, supporting their independence though prevention and reduction of need. You will hold a caseload of OT cases and provide specialist OT consultation to the wider team.
The OT process within the team is as follows:
Assessment: Using both home grown and standardised MOHO based assessment tools you will formulate a clear understanding of the client’s individual occupational strengths and challenges. As part of your assessment you will consider the client’s readiness for change to enable you meet them at their current functional level.
Goal Planning: The findings of your assessment will help identify the foci of your interventions and so inform your client centred and SMART goal planning.
Interventions: You will design creative and client centred interventions that support clients to move forward towards their goals. You will work closely with the client and their networks to maximise their opportunities for success. Your work will be predominantly based within the community; supporting clients to function more effectively both within their home environments and their local communities enabling them to participate in educational, vocational and social resources available. You will use the outcomes of your OT assessment and intervention to help inform reablement focused care plans of social care support.
Generic functions: You will be responsible for a smaller caseload of generic review cases; reviewing the clients social care needs under the Care Act 2014 to ensure that their care needs are being met in by reablement focused social care provision. You will undertake duty as part of the team rota and develop you skills in safeguarding, acting as safeguarding investigator when required.
The team currently has a hybrid model with regard to remote and office based working, which is under continuous review based on changing landscape of the pandemic and needs of the services.
We have a wide range of CPD opportunities to develop your OT skills including but not limited to access to formal training, regular professional supervision and shared CPD sessions with SLAM and the wider Adult Social Care OT provision. You will also have access to a wide variety of generic training and the opportunity to train as a Best Interest Assessor (BIA) or an Approved Mental Health Professional (AMHP).
About you:
We are looking for a self-motivated and experienced mental health Occupational therapist.
You will be passionate about providing high quality client centred care to support adults with mental health difficulties to maximise their wellbeing and independence within the community. You will have a working knowledge of the MOHO and the Care Act 2014 and a commitment to continuing professional development.
