The Role:

At Intermediate Care Southwark (ICS), we are an integrated health and social care team comprising dedicated professionals such as physiotherapists, social workers, occupational therapists, nurses, pharmacists, and more. Together, we provide short-term, targeted support to maximize individuals' independence and promote recovery. Our focus is on Reablement—optimising skills to support recovery and independence. We work from a strength-based approach, building on existing skills and resources, while also collaborating with specialist care providers.

Our team has recently achieved significant milestones in promoting independence and recovery among our service users. Through our collaborative efforts, we have successfully facilitated numerous individuals' transitions to greater independence and improved quality of life.

About You:

As a Customer Support Officer you will have the opportunity to work in a dynamic, Multi-Disciplinary Team setting, focusing on strengths-based practice and promoting independence and recovery.

Your main duties will include:

- Providing professional expertise, skills and advice to ensure the delivery of high-quality services within Adult Social Care.

- Collaborating with Multi-Disciplinary Team Managers and reporting to Senior Business Support Officer to deliver a safe, strength-based service, promoting and improving independence

- Engaging in safeguarding functions, including escalating concerns to minimise risks to safety and well-being.

Experience and Skills:

- Demonstrated ability to build rapport with clients and families, effectively communicating and understanding their needs.

- Proficiency in identifying risks during client interactions and referring clients to appropriate resources or escalating to duty managers when necessary.

- Skilled in obtaining feedback from care agencies and effectively managing complaints against our care provision

- Confidence in handling systems, referrals, and communication with various healthcare professionals.

- Adaptability to change and proficiency in managing spreadsheets, inputting referrals, and maintaining statistics.

- Strong advocacy skills, supporting clients and explaining complex policies such as the Charging Policy.
