The role:
The OPPD Service supports older adults and adults with a physical disability. We are a multi-professional service comprising of Social Workers, Occupational Therapists, and Assistant Practitioners. We are recruiting to an Assistant Practitioner post within our Assessment and Intake (A&I) and Transfer of Care (TOC) Teams.
A&I provides a six week assessment and care planning service for services users based in the community and TOC supports service users who have recently been discharged from hospital requiring further monitoring, assessment and review.
Within both teams you will be responsible for completing Care Act 2014 assessments to determine whether or not an individual is eligible for services. This includes the undertaking of proportionate assessments, assessing ongoing support, support planning and proactively working with complex issues, family dynamics, legal issues, crisis, providing information and advice, transferring to another team if required and the provision of some ongoing safeguarding work, under direct supervision from a qualified professional.
You will also be asked to complete Carers assessments to ensure that we are supporting the numerous informal carers within the borough.
