The Role:

We are looking for an enthusiastic and creative Assistant Practitioner to join Southwark's Mental Health Adult Social Care department as part of the Mental Health Reablement Team.

The Mental Health Reablement Team is a short-term service that aims to prevent and reduce social care needs whilst promoting independence and recovery within the community. It offers a targeted and brief intervention to enhance service user wellbeing and independence, which avoids the need for longer-term social care support.

As an Assistant Practitioner in the team the post-holder will support the Social Workers and Occupational Therapist by aiding them in implementing support plans and supporting service users to achieve their goals. There are opportunities for the post holder to develop their skills and confidence so they can hold their own caseload and plan and provide interventions and support. Work with service users takes place in the team base, community and in their homes, where appropriate.

About You:

To be successful in your application we expect you to have an understanding of the principles of reablement and the impact of mental health issues, as well as experience of implementing support plans and supporting service users in a person centred way.
