The Role:

We are recruiting Assistant Practitioners - Occupational Therapy (OT Assistants) within our Intermediate Care Southwark (Reablement) Service. Intermediate Care Southwark is an integrated health and social care service which provide both Urgent Response and Rehabilitation and Reablement pathways to residents of the London Borough of Southwark.

To help prevent and reduce social care needs, whilst promoting recovery and independence within the community. To deliver and monitor therapy plans which maximise independence, wellbeing and resilience within the community.

Provision of practical assistance in terms of the provision of disability equipment and home adaptions to clients in order to maintain maximum independence within the community.

To work from a strength based position, building of individual skills and resources, and strengthening links with community preventative services.

About you:

We are looking for applicants with:

  • Experience of working in health and social care or rehab settings.
  • Experience of working with multiple agencies or professionals.
  • Experience of assessing the needs of elderly people / people with disability.
  • Numeracy and literacy skills with ability to write factual and accurate reports
  • Understanding of current issues affecting health and social care.
