The role
We are looking for enthusiastic, committed and creative Assistant Practitioners to join Southwark’s Mental Heath Adult Social Care division.
The service offers assessment and support to adults whose mental illness and/or impairment restricts their activities of daily living. Our focus is to maximise a person’s independence, to offer choices, to promote wellbeing and to meet our duties under the Care Act. We have the scope to develop creative and innovative approaches to ensure service users are at the centre of the support they receive.
We work closely with primary care, secondary mental health services and third sector providers. Working in collaboration with the service user, you will enable current support plans to be implemented on both a practical and emotional level. You will ensure that such plans are personalised, recovery focused and promote independence.

We are looking for two Assistant Practitioners to join each of the following teams:
1 x Grade 8: Mental Health Reablement Team
Implementing support plans and interventions to adults within the community or who are being discharged from psychiatric hospital to ensure needs are being met, choice, independence and wellbeing are being promoted and to prevent delays to discharging people from psychiatric wards.

1 x Grade 8: Mental Health Move On Support Team
Implementing support plans and interventions to adults placed in various mental health accommodation (supported, residential and nursing settings) to ensure needs are being met, choice, independence and wellbeing are being promoted. Part of the role is to support service users to move to alternative accommodation and to support with resettlement on both a practical and emotional level. The post holder may need to travel outside of London to visit placements.
