The Role:
This role has a key leadership role in integrated commissioning in Southwark. It is jointly funded by Southwark Council and South East London Integrated Care System. Therefore, the role is a joint role within a joint programme team aligned to the shared objectives of the two organisations.
The postholder will have a significant role in building working relationships and collaboration between Partnership Southwark partners.
As part of this team the postholder will lead and therefore significantly contribute to the delivery of a specified programme of work which will focus on a defined population within Southwark.
The postholder will be expected to contribute to the development of the team in Southwark by leading a core team to co-ordinate the delivery of a complex programme of work centred around delivery of the core priorities for both organisations.
The postholder will also significantly contribute to planning for continuous reduction in health and wellbeing inequalities across the borough in collaboration with the wider system through Partnership.
About you:
Play a leadership role in developing integrated pathways, facilitating and promoting innovation and transformation to improve outcomes. Leading the work with key stakeholders and professional leads including clinicians to identify needs and priorities for populations, in line with local regional and national policies.
Provide senior leadership to the transformation and development of children and young people and adults and older adults through the development of new models of care and any associated contracting arrangements.
Provide strategic leadership to the local commissioning strategies/plans and support the senior leaders of both the council and SELICS (South East London Integrated Care System.
Provide leadership in the delivery of the agreed joint commissioning scope between the ICS and the Council.
Work collaboratively with peers within both organisations to encourage the sharing of good practice and interdependencies between the programme are identified and planned for.
Using the Bridges to Health and Wellbeing methodology lead the development of measurable outcomes for the population programme and hold a key contributory responsibility for their delivery.
Provide expertise and leadership in translating relevant national, regional and local policy and direction associated with developing, transforming and improving local services and support for residents of Southwark.
Lead and develop a much stronger approach on prevention and targeted early action as well as deeper integration across health and social care, and wider council services, including adult social care, housing, continuing care and quality and performance.
Take a key role in both the delivery of a population based transformation programme and support the delivery of aligned programmes within Partnership Southwark.
Contribute to agendas of both the Council and SELICS.
