The role
All Age Disabilities Service works with service user 0-25. Learning Disabilities works with service user 25+This services provide a comprehensive and integrated social care pathway for young people and adults with learning disabilities and other disabilities using the Adult legislation and care and support.
This includes the undertaking of proportionate assessments, assessing ongoing support, support planning and proactively working with complex issues, family dynamics, legal issues, crisis, providing information and advice, transferring to another team if required and the provision of some ongoing safeguarding work, under direct supervision from a qualified professional.
You will also be asked to complete Carers assessments to ensure that we are supporting the numerous informal carers within the borough. A key part of this role is to develop trust and assist in safeguarding and promoting positive and timely outcomes for service users and their families. Ensuring focused interventions are delivered to safeguard and empower our service users with learning disabilities.

About You
We are looking for resilient practitioners who can manage competing demands and prioritise work effectively. The role requires applicants who are confident decision makers and who can stay calm under pressure. You will have the ability to undertake strength based social care assessments. You will be able to determining the need of an individual and produce outcome focused support plans.
You will have the ability to establish a rapport and build and maintain a professional, respectful and honest relationship with the service user and their family/carer.
You will have the ability to deliver focused interventions to safeguard vulnerable adults and promote independence, positive outcomes and promote the use of assistive technology.

• 36 hours Monday - Friday
• This is a x1 post permanent position and 1 fixed term post 18-24
• Enhanced DBS check is required for this role
