The Roles:

Southwark is recruiting 2 x Social Work Deputy Team Managers: One position is in the OPPD Contact Team and second position in the Transfer of Care Service.

The successful candidate will be required to provide a management function in our fast-paced OPPD Contact Team, working closely with service users and a range of other professionals across a wide variety of settings within Social Care, Health, Housing, plus the voluntary and independent sector.

Both services support adults with physical disabilities, sensory needs, dementia and a variety of often complex health conditions, using strengths based approaches to enable all Southwark residents to reach their full potential.

The OPPD Contact team is the first point of contact for community referrals and works with partner agencies in providing a universal offer of support, information and advice and connecting residents to their wider communities.

The Transfer of Care (TOC) service is predominantly hospital based and provides Southwark’s hospital discharge function. It supports adults to leave hospital in a safe and timely way, offering relevant support to residents, families and health partners to manage effective flow out of acute hospitals back into the community or into other rehab or placement settings. Note: this post is based at St Thomas hospital.

Both posts are within Southwark Adult Social Care and report directly to the relevant Team Manager for each of the respective service areas.

Both roles cover a range of line management responsibilities working closely with health, housing and voluntary sector colleagues and responsible for setting the standard and supervising the work produced by the team’s social care staff, with day to day management covering;

covering:

- Safe and timely hospital discharges (relevant to TOC Service post)

- Safeguarding decision making including triaging safeguarding referrals and implementing immediate protection plans

- Mental Capacity, DoLS and Best Interest decision making

- Developing and co-ordinating approaches to enable the delivery of services in an efficient and effective manner and in accordance with departmental policies and procedures.

About you:

You will be a registered Social Worker and have experience of managing teams within Health and/or Social Care. You will have a genuine passion for the provision of high quality services and demonstrate a commitment to asset-based ways of working. Please note the successful candidate will require a robust understanding of Safeguarding procedures and practices.

You will have awareness of current Social Care legislation and social policy and understand the principles of Equality and Diversity and its practice.

You will be a practical thinker and have the ability to manage competing demands and ensure we put the person at the centre of all that we do.
